Each Source Data Provider (SDP) is given the rights to access their own data and enter remotely at their site via the EYEGP Image Reporting System (EIRS).

    How to access the EYEGP Image Reporting System (EIRS)

  1. Register your centre as a Source Data Provider (SDP) with the Core Team.
  2. Fill in the Authorisation List form for the purpose of access control to receive your username and password. Click here to download Authorisation List form (in PDF Format) .
Security
  1. The Authorisation List must be signed and chopped by the Site Investigator.
  2. Please submit the completed form via Feedback page.

Each authorised user is required to read through and understand the Security Policy for EYEGP Image Reporting System (EIRS) before being authorised to access the application. This is to ensure that users play an active role in ensuring security of data at their level.

Security Practice Guideline

As a good security practice, you are strongly advised to:

Keep your password confidential!

  • Avoid sharing or divulging your Password to anyone. This includes any person who may appear to represent or work for the EIRS. Our administrators do not request your password at any time.
  • Avoid using the same Web Application Password for any other web-based services such as for email or for Internet Service Provider login.
  • Avoid choosing a Password that is easily anticipated by a third party, like your NRIC number, telephone number, date of birth, etc. You should select a unique Password to make it difficult for anyone to anticipate.
  • Avoid writing down or "saving" your Password on your browser or any other software. Memorize your Password.
  • If you suspect your Password may have been compromised, change your Password immediately.

Tips: Your password is what tells the system that you are who you say you are. Because your password is like a key to your account, you need to safeguard it. Anyone who has your password can pose as you. Therefore, you may be held responsible for someone else's actions if they can get your password. Do not record your passwords down, memorize them!

Ensure you are accessing the correct website!

  • Never access the website via a hyperlink from an email. Always enter the correct website address yourself, which is eyegp.com and click the EIRS app link.

Only access Web Application using a secure and trusted computer!

  • Never access your Web Application on computers/devices about which you have doubts regarding security, such as those located in public places. If you have to use such computers (for example, when you are outside the office), change your password once you have access to a secure computer.
  • Keep your operating system (e.g., Microsoft Windows) and Internet-related software updated with the latest security patches.
  • Protect your computer from viruses and malicious programs with antivirus software and firewalls where possible. Always update your antivirus software with the latest virus signatures.
  • Always log out of your Internet session by clicking on the "logout" button whenever you leave your computer, even for a short while. Do not simply close the browser window when you wish to end the Web Application session.
EYEGP Image Reporting System (EIRS) Information Security Policy

Introduction

This document has been produced to ensure that:

  • A higher level of security in terms of patients' information confidentiality, centre confidentiality, integrity, and availability of data information is maintained at all times.
  • All sites users and site coordinators are aware of their authority and accountabilities as stated in the EYEGP Image Reporting System (EIRS) Authorization Form.
  • All site users are aware that EYEGP Image Reporting System (EIRS) is governed and operated based on various approvals and policies such as Good Clinical Practice, Personal Data Protection Notice, and Privacy Policy which are available on the EYEGP Image Reporting System (EIRS) Website and thus should ensure compliance with the stipulated policies.

Management

A) User Level

  • Provide true and up-to-date information about yourself during registration on the MyProfile page.
  • Do not disclose your user ID or password to anyone else. Each activity in the web application has an audit trail.
  • Do not give your mobile phone to anybody else while logging into the web application.
  • If you lose your mobile phone or there is a change in your mobile phone number or email address, please inform the Site Coordinator / Doctor in-charge who shall then officially inform the EYEGP Image Reporting System (EIRS) Core Team / User Manager via the Feedback page.
  • If you no longer serve at the current site, kindly inform the Site Coordinator / Doctor in-charge as well as the EYEGP Image Reporting System (EIRS) Core Team / User Manager via the Feedback page to deactivate your account.
  • If the you have forgotten your password, please click the Forgot Your Password on the login page of the web application. It is compulsory for the user to provide the correctly registered login email address for verification. The password will be sent to your email. If you did not receive the email within 30 minutes, please check your spam box and if it is still not there, please contact the IT administrator of EYEGP Image Reporting System (EIRS).
  • Please read password management as per Security Practices.
